General Information
Appointments
Patients are seen by appointment. A physician's referral may be required by your insurance carrier before our doctor can see you. Patients may be seen in any of our three offices. Noninvasive vascular testing is performed in the downtown office only.
Telephone Calls and Emergencies
Emergency calls are answered 24 hours a day, seven days a week. Callers with non-emergency business are encouraged to telephone during regular business hours.
Prescriptions
For prescriptions, call your pharmacist. The pharmacist will call our office directly for the doctor's authorization. Please telephone during regular office hours, when your record is available for review. Please plan ahead to avoid running out of medication. Narcotic pain medication will not be refilled when the office is closed.
Hospitalization
When hospitalization is necessary, we utilize the following facilities: Jewish Hospital, Norton Downtown Medical Center, Baptist Hospital East, and Norton Suburban Hospital.
Insurance
Our staff is happy to assist you in completing insurance forms, and we accept most major insurance plans. Please bring your insurance card with you for all office visits and inform us of any changes in your insurance coverage.
Vascular Laboratory
We maintain a state-of-the-art noninvasive vascular laboratory in our main office downtown in the Rudd Heart & Lung Building. Patients can often receive diagnostic testing the same day of their office visit. Appointments in the vascular laboratory are scheduled with a doctor's order.
Disability
Our staff is happy to complete disability and indemnity forms. A $10.00 processing fee is charged for each form, payable when the request is submitted.
