Your First Visit
Upon arrival, you will be asked to complete the patient registration and medical release form. This is pertinent information, which you will be requested to complete entirely as it will become a permanent part of your medical record. Please have your insurance card(s) and picture identification available so that a copy can be made for our files.
Canceling an Appointment
If you unable to keep an appointment, please call as soon as possible to cancel or reschedule. Because of emergencies, we may need to reschedule your appointment. We regret that this may happen without advance notice.
When Questions Arise
When new matters relating to appointments, payments, policies or business matters occur, please call (478) 923-0144. Our office will be able to answer your questions and assist you.
Why We Practice As Associates
We feel that a group practice is beneficial to you and us. In this way we can:
- Provide coverage to you 24 hours a day.
- Assure you that if your doctor is away, the other doctors have access to your records.
The on-call doctor is as interested in your well being as your own personal doctor.
Referrals
No referrals are necessary unless required by your insurance carrier. If you are referred, the physician who referred you will promptly receive a letter outlining the results of your evaluation and treatment plan.
Our Office Hours
The office is open from 8:00 a.m. until 5:00 p.m. Monday through Thursday and 8:00 a.m. until 4:30 p.m. on Friday.
Outpatient Surgical Procedures
Certain surgical procedures, which once required hospitalization, can be safely performed today in our office. Most procedures that can be performed under local anesthesia can be done in our state-of-the-art office surgical suite. Some of the more common procedures include breast augmentation, vasectomy, laser treatment for varicose veins, breast biopsy, excision of cysts and treatment of certain skin lesions.
