New Patients
Please arrive 15 minutes before your appointment bringing with you the following:
Photo Identification Card
Insurance Card (if applicable). We must have your current insurance card in order to file your claim.
Current Medication List
Filled-out New Patient Forms. Please download here.
Method of payment - We collect all co-payments and/or deductibles at the time of your visit. For your convenience (in addition to cash/checks) we accept Visa, MasterCard and Discover.
Attention Female Patients: Please do not wear any make-up to your visit. This will allow Dr. Possick to examine your face for skin cancers or other skin conditions. For your convenience, we have free face wash kits at our front desk.
We understand that from time to time you may not be able to make your scheduled appointment. We ask that you please give us a 24 hour cancelation notice on all cancelations/reschedules. IF you do not give us a 24 hour notice a $20.00 cancelation fee may apply. It is also at Dr.Possick’s discretion if he will allow us to reschedule your appointment.
