Who is Eligible for Our Discounted Services?

Park DuValle Community Health Center, Inc. is a Section 330 funded federally qualified health center.  Sliding fee discounted services are offered to residents of  Jefferson, Spencer, Anderson, Bullitt, Henry, Nelson and Shelby Counties of Kentucky. Eligibility for sliding fee scale discounts is based on county of residence, income and household size.  Written documents verifying income and residency must be presented as part of the application for sliding fee discounted services. Patients who do not meet income or residency guidelines for sliding fee discounted services or who prefer to receive services without providing income/residency documentation may be served at full fee/undiscounted rates or by using their public or private insurance plans.  Please check with us by calling (502) 774-4401 or via the "Contact Us" section on our website to determine if your insurance can be accepted at our sites or to get more information on documentation we will accept for sliding fee discount applications. 

All sites are licensed Primary Care Centers. The main site for Park DuValle Community Health Center is located at 3015 Wilson Avenue. Services offered at the main site include: preventive health services, primary adult and pediatric medical care, women’s health, family planning, ophthalmology, primary dental care including dental hygiene, social services, nutrition counseling, immunizations, pharmacy, x-ray, laboratory, Women, Infants, and Children’s (WIC) Program, and transportation services. Mammography is offered one week per month in collaboration with the Partnership for Cancer Control. Free health education, disease management, smoking cessation, life skills and wellness classes are offered to patients and members of the broader community. Our three satellite sites directly offer preventive and primary health and medical care services and other services including pharmacy and dental services by referral to the main site.

On weekends the main site is open to adult and pediatric patients on Saturdays from noon until 4:00 pm with one primary care physcian and nurse on site.  Pharmacy and Dental are not open on weekends.

Joseph and Joseph, Architects, designed the main site of Park DuValle Community Health Center located at 3015 Wilson Avenue in Louisville, KY to meet the health care needs of the community now and far into the future.  Joseph & Joseph Architects also designed the Park DuValle at Newburg and Park DuValle at City View Health Centers.  Their classic designs and use of color has created an aesthetically pleasing and culturally appropriate setting for the delivery of quality health care to a deserving community.

G. C. COXE AND KEITH JOHNSON MURALS

The colorful murals shown in the right columns of the Park DuValle Community Health Center website are original artwork created by renowned Louisville, Kentucky artists, Gloucester Caliman Coxe (1907-1999) and Keith Johnson.  Photographs were taken of actual staff and patients at Park DuValle Community Health Center and used to create these unique visual images of our caring staff and beloved patients. The original works are on display at Park DuValle Community Health Center’s main site where they serve as a historical tribute to former staff and patients and as a constant reminder that “We are all about caring”.
