Who is Eligible for Our Discounted Services?
Park DuValle Community Health Center, Inc. is a Section 330 funded federally qualified health center. Sliding fee discounted services are offered to residents of Jefferson, Spencer, Anderson, Bullitt, Henry, Nelson and Shelby Counties of Kentucky. Eligibility for sliding fee scale discounts is based on county of residence, income and household size. Written documents verifying income and residency must be presented as part of the application for sliding fee discounted services. Patients who do not meet income or residency guidelines for sliding fee discounted services or who prefer to receive services without providing income/residency documentation may be served at full fee/undiscounted rates or by using their public or private insurance plans. All sites are licensed Primary Care Centers. The main site for Park DuValle Community Health Center is located at 3015 Wilson Avenue. Services offered at the main site include: preventive health services, primary adult and pediatric medical care, women’s health, family planning, ophthalmology, primary dental care including dental hygiene, social services, nutrition counseling, immunizations, pharmacy, x-ray, laboratory, Women, Infants, and Children’s (WIC) Program, and transportation services. Mammography is offered one week per month in collaboration with the Partnership for Cancer Control. Free health education, disease management, smoking cessation, life skills and wellness classes are offered to patients and members of the broader community. Our three satellite sites directly offer preventive and primary health and medical care services and other services including pharmacy and dental services by referral to the main site.
On weekends the main site is open to adult and pediatric patients on Saturdays from noon until 4:00 pm with one primary care physcian and nurse on site. Pharmacy and Dental are not open on weekends.
