Patient Forms

If you are a new patient to Drs. Padgett, Allen & Cates Dentists, or you need to update your information, for your convenience we provide our patient forms to download and print. Please bring them with you on your first or next visit or call the office if you have any questions.

We accept all dental insurance including Medicaid, and we will file your claims for you as an extra service. If you have specific questions about your coverage or benefits, please let us know and we will do our best to answer!
