For New Patients
We welcome new patients and encourage you to make an appointment for a comprehensive dental exam. For your convenience, our new patient forms are available online to print and complete before your first visit.
We will gladly submit to your primary insurance company for all treatment rendered and accept direct reimbursement from the insurance company. Co-payments for amounts not covered by your insurance will be due when services are rendered. Any pre-treatment estimates will be submitted to provide you with the most accurate information regarding your coverage and co-payments. All insurance information is entered into our computer system and certain claims are sent electronically to expedite processing payment. We are a preferred provider with Aetna PPO, Cigna PPO, Metlife PPO, and Delta Dental PPO.
