Admission Procedures

Admissions Information and Policies

Application Procedures

We welcome and appreciate your application to Heritage Preparatory School. The admission procedure for your child will include the completion of the following items:

  • Obtain an application package from the registrar's office.

    •
  • Schedule an appointment for a personal tour of our campus.

    •
  • Turn in the following completed forms to the registrar's office:
    - Application for Enrollment Form
    - Statement of Cooperation
    - Day Care Registration (if applicable)

    •
  • Make an appointment to have the applicant(s) tested.

    •
  • Turn in the following to the receptionist:
    (Please be sure to clearly label these forms with the student's name and grade.)
    - Updated physical form (HRS-3040)
    - Immunization records (HRS-680)
    - Copy of Birth Certificate
    - Copy of applicant's last Report Card (Grades 1-12)
