Dr. Hazen

Born and raised in the Daytona/Ormond area, Dr. Hazen is a second-generation dentist. Dr Hazen’s father practiced dentistry from 1961 to 1998. Dr. Hazen joined his father’s practice in 1991 and continued upon his retirement in 1998. Dr. Hazen and his staff are committed to running a dental office that is focused on patient care. Each team member believes in providing patients with the same quality of care and personal service that they would give their own families.

We accept any insurance that allows you to choose your own dentist. We will verify and file your insurance claim as a courtesy to you. Any portion that insurance does not cover is the patient's responsibility. We have financing options available if needed. We offer Care Credit and Capital One for your financing needs.
