Payment Terms Payment for services is due at the time services are rendered. We accept cash, checks, MasterCard, Visa, American Express, Discover, and Health Care Credit.* A 10% discount will be extended to patients pre-paying for full mouth/ total treatment plan. Ask our office manager for details. Returned checks may be subject to a penalty charge. We reserve the right to charge for broken appointments and appointments canceled without 24 hours advance notice.

Insurance
We will be happy to file insurance for you as a courtesy. We will need a copy of your insurance card. You must also realize that your insurance is a contract between you and your employer and the insurance company. We are not a party to that contract. All charges are your responsibility from the date the services are rendered. Also note that not all services are a covered benefit in all insurance contracts. If you have insurance, we require you to pay your portion of the charges as a down payment at the time of service. You will be asked to pay for adult fluoride treatments and sealant treatments at the time of visit, unless your insurance covers these procedures.

Services Requiring Laboratory Expenses
We ask that you pay a minimum deposit of 50% at the time of impression, whether you have insurance or not. The balance is to be paid at the time of placement. Laboratory expenses are incurred for, but not limited to, crowns, bridges, partials, dentures, etc.
