New Patient Visit Checklist
- Completed new patient forms (Arrive 15 minutes prior to appointment if forms are not already completed.)
- Photo ID
- Insurance card(s)
- Payment source for co-payment/co-insurance/deductibles/payment on existing balances
- Always bring all prescriptions with you to each office visit.
Established Patient Visit Checklist
- Photo ID
- Insurance card(s)
- Payment source for co-payment/co-insurance/deductibles/payment on existing balances
- Always bring all prescriptions with you to each office visit.
- If it has been more than one year since your previous visit, then we may ask you to re-complete a new patient form. See New Patient Form on left side of this page.
