Current patients are encouraged to arrive 15 minutes before their annual physicals to complete the physical questionnaire. You also have the option to print the form from our website to bring with you to your appointment.
To print, you can right click on the link and choose "open." Open the file on your computer and print. You can also save the file to your computer by right clicking on it and choosing "save target as," opening it from your computer, and printing. If using the fillable version of the form, be sure to print the form before you close it - the form is unable to save your answers. There is a different form for men and women, so be sure to pick the right one.
