INSURANCE AND CO-PAY
We understand that insurance details can sometimes be confusing. Our staff will be happy to help you file insurance claims and answer any questions that you may have, so please don't hesitate to ask. And remember, whenever you visit our office, please bring your insurance I.D. card, and be prepared to pay your co-payment and any deductible remaining at the time of your appointment.
HOURS AND LOCATION
Our hours are Monday through Thursday from 8:30am until 2:00pm. Our office is located at 5315 Sunset Boulevard, also known Highway 378, across from Uno Chicago Grill in Lexington.
