CFP CARE TEAM – WHO ARE WE?

A division of CFP Physicians Group involves medical care for patients in nursing homes and assisted living facilities. CFP Care Team provides quality medical care in more than 20 area extended care facilities. We strive to provide quality medical care and ancillary services at the patients’ bedside, eliminating unnecessary trips to the physician’s office. The Care Team is composed of our physicians, advanced registered nurse practitioners (ARNP’s), physician assistants (PA’s) and podiatrists all of whom specialize in Geriatric medicine. Our team serves as Medical Directors of numerous Skilled Nursing Facilities and local Home Health Companies providing extensive knowledge in the administrative side of medical care.

Our mission is to manage your daily care, treat you with dignity, respect your decisions and coordinate your rehabilitation.
The Care Team professionals will make regular visits to evaluate your progress. Your Care Team professionals are available to talk with you and your family as needed.

The goal of your rehabilitation is to reach your maximum potential. Your progress will be evaluated on a weekly basis. The Care Team professionals work closely with the rehabilitation facility to determine a safe and appropriate time for your discharge home.

The discharge planning begins when you are admitted to a Skilled Rehabilitation Center after an acute hospitalization. During this process it is important to educate the patient and family regarding the patient’s needs (equipment and medication). A Home Health Company is often consulted to provide continued monitoring when discharged home. Follow-up with your primary care physician and specialist are encouraged.
