Please include your name, date of birth, and contact number(s). Include name of provider who is to receive message.
For prescription refills include name of medication, strength and directions. Include pharmacy name and phone number.
For x-ray and lab results include when and where test was done.

Monday- Thursday 7:30am-6pm and Friday 8AM-6PM, Every other Saturday Alternating Hours: 9am-12pm and 9am - 4pm., respectively.
Lab Hours
Monday-Friday 7:30AM-5PM and Saturday 7:30AM-12noon.
