Emergencies
For life-threatening situations, call 911. If you have an urgent problem, please call our office at 615-370-7905. If you reach the recording after hours, press zero (0) to be connected to the answering service operator.
Test Results
A medical assistant will call you with test results after Dr. Butterfield reviews results and writes her assessment and recommendations. If you have had tests done and have waited more than a week for results, please call 615-370-7905 to alert us that you are still waiting to hear about test results.
Prescription Refills
We respectfully request that you see Dr. Butterfield yearly for prescription renewals. You will be given your prescriptions at this appointment. Your prescriptions will typically be refillable for one year unless Dr. Butterfield needs to follow how often the medication is used. All refill requests made before 4:30 pm will normally be completed the same business day unless you have not seen the doctor for over a year. As a courtesy to you, we will call and leave a message that the prescription has been called in.
Appointments
Routine appointments can be scheduled by phoning 615-370-7905. Schedule follow-up appointments as you leave or by calling 615-370-7905. We make every effort to stay on schedule, although emergencies do occur occasionally. As a courtesy to other patients and staff, please call the office as soon as possible if you are unable to keep your appointment or are going to be late. If you are 15 minutes or more late we will make every effort to work you back into the schedule as long as it does not displace other scheduled patients. If the schedule that day will not allow a work-in, you will be asked to reschedule your appointment in fairness to all our patients.
For urgent appointments, please call our office at 615-370-7905. We allow certain appointment times during the morning and afternoon for work-in appointments and make every effort to see patients the same day or within the next couple of days. For your convenience, you may download patient forms by using the links below. This will help expedite the patient registration process.
NEW PATIENT FORMS
Insurance Card(s)
We request that you allow us to see your insurance card(s) at each visit. Because of the constantly changing insurance environment, we have found that this is the best way to ensure that we have the most updated insurance information on our patients and thus avoid unnecessary delays in filing your claim.
